Getting a Handle on Piglet Diarrhea: Why Testing for Five Gut Pathogens at Once Makes Sense

Anyone who's worked with farrowing or nursery pigs for a while knows the scene. Piglets are scouring. Some look rough. You try one treatment, then another. Nothing really works. Or it works for a day, and then things fall apart again.
Most of the time, the problem isn't one single bug. It's two, three, or even four of them working together.
That's where a combo test - looking for rotavirus, C. diff, Cryptosporidium, E. coli, and Clostridium perfringens all from the same poop sample - changes the game.
Why one bug is rarely the whole story
Go back ten or fifteen years, a lot of folks thought piglet diarrhea was usually just E. coli or maybe rotavirus. Give some antibiotics, or use a vaccine in the sows, and you'd get by.
But we've learned more since then. And the diagnostic tools got better. Now we know that mixed infections are actually the norm, not the exception.
Some numbers from field studies: well over half of diarrheic piglet cases involve at least two pathogens. Sometimes more.
Here's how it plays out in real life.
Rotavirus goes first. It trashes the intestinal villi - those little finger-like things that absorb nutrients. Once that barrier is damaged, E. coli and Clostridia move in like they own the place. They don't even need a big dose to get started. The door's already open.
Or take Cryptosporidium. This parasite doesn't get as much attention as the others, but it's everywhere once you start looking for it. It scrapes up the gut lining, and suddenly bacteria that the piglet used to handle just fine turn into a real problem.
So if you only test for E. coli and it comes back positive, you might think "ah, that's it." But you could be missing the virus or the parasite that set the whole thing up. And that's why the treatment fails.
The trouble with traditional testing
Most vet labs can run individual tests for each of these pathogens. But there are practical problems.
First, time. Bacterial culture takes days. Virus isolation is even slower. Parasite microscopy? Depends a lot on who's looking through the lens. By the time you have all the results back, half the litter could be gone or you've already gone through three different treatment protocols.
Second, cost. Running five separate tests adds up fast. On a tight margin operation, producers skip testing and guess instead. We all know how that usually ends.
Third, you can't really do it on farm. You're packing up samples, shipping them somewhere, waiting on the phone call. For a fast-moving problem like C. perfringens type C, waiting 48 hours is not an option.
What a five-in-one combo test actually does
The combo test is just a simple dipstick - same kind of technology as a rapid COVID test or a pregnancy test. You take a fresh fecal sample, mix it with the solution they give you, and dip the strip.
Thirty minutes later, you see lines. Each line tells you yes or no for one of the five pathogens.
That's really it. No incubators. No microscopes. No waiting on the lab.
Someone on your crew can learn to run it in about ten minutes.
Where people get the most use out of it
Outbreak response. This is the obvious one. Pigs are scouring hard. You grab a few fresh samples from untreated piglets - key word is untreated because if they've already gotten antibiotics or fluids, you might get false negatives. Run the combo test right there in the office. If rotavirus lights up but E. coli doesn't, you just saved yourself from pouring antibiotics down the drain. If C. difficile is positive, you know supportive care and gut protectants are your main play. If it's E. coli and C. perfringens both positive, you're probably looking at a different approach.
Sow side screening before farrowing. This one's underused, in my opinion. A sow that's shedding C. difficile or Cryptosporidium into the farrowing crate is basically setting up her own piglets for failure. You can test a fecal sample from the sow a few days before her due date. If she's positive, you can adjust your management - extra cleaning, maybe separate the piglets earlier than usual, or use a different disinfectant (some work better on parasites than others).
Weaning time monitoring. We all know weaning is a stress bomb. The pigs are changing diets, moving to new rooms, mixing with other litters. It's prime time for E. coli and rotavirus to flare up. Running combo tests on a few sentinel pigs right before weaning or right after can tell you what's simmering before it boils over.
How to read the results without overthinking it
A positive line means the antigen was there in the sample. That's all. It doesn't automatically mean that bug is the main problem.
Context matters.
A strong, dark band for rotavirus in a four-day-old piglet with watery diarrhea? Yeah, that's your culprit.
A faint, barely-there band for E. coli in a three-week-old pig that's eating fine and has normal feces? Probably ignore it. Most healthy pigs carry some E. coli.
And remember - just because you see two positives doesn't mean you treat both. Figure out which one started the mess and which ones are just taking advantage of a damaged gut.
One more thing: combo tests are fantastic for ruling things out. A negative result is very reliable. So if your test says no rotavirus, no C. diff, no Cryptosporidium, you can stop chasing those and focus on bacteria or something else like TGE or PDCoV.
Where it fits in a bigger control plan
The test alone won't fix your diarrhea problem. It just tells you what you're dealing with. You still have to do the other stuff right.
Biosecurity. Clean farrowing rooms. Hot water, good detergent, then a disinfectant that actually works on the bugs you found. Cryptosporidium laughs at a lot of common disinfectants, by the way. Hydrogen peroxide or ammonia-based products are better for that one.
Colostrum. Nothing replaces it. Piglets born without good colostrum intake are sitting ducks for every pathogen on this list. Get them on the sow early, or have a colostrum replant on hand if needed.
Vaccination. Works for some of these, not for others. Rotavirus vaccines in sows help. C. perfringens toxoids help. But you're not going to vaccinate your way out of Cryptosporidium - at least not yet.
Environment. Cold drafts and damp floors make everything worse. A piglet that's cold is an immunosuppressed piglet. Fix the barn conditions and some of your "disease problems" will quietly go away.
A final thought from the barn
If you take away one thing from this, it's that piglet diarrhea is almost never simple. The days of assuming it's just one bug are over.
A five-in-one combo test won't solve all your problems. But it'll stop you from guessing. And in this business, guessing is expensive.
Run the test. Read the lines. Treat what matters. And fix the management holes that let the bugs stick around in the first place.
