Canine Leptospirosis – A Clinician’s Take

Honestly, leptospirosis is one of those diseases that keeps you on your toes. It's bacterial, it's tricky, and it doesn't play nice-kidneys, liver, sometimes worse. And yeah, it's zoonotic, so every time I see a suspect case, I'm also thinking about the owner standing right there in the exam room.

 

Transmission? It's all about urine and water.

Wildlife carries it-raccoons, skunks, rats, you name it. They pee, the bacteria hang out in moist soil or standing water, and dogs just happen to love sticking their noses into exactly those places. High-risk dogs? Definitely the outdoor adventurers, the hunting dogs, the ones that treat every puddle like a drinking fountain. Rodent-heavy areas are a red flag, too.

 

What do you actually see?

That's the annoying part. Early on, it looks like a hundred other things: fever, lethargy, not eating, maybe some GI upset. Owners often say, "He's just off." Sometimes there's increased thirst or peeing-that's a clue, but not always there.

Once it digs in, though, the picture changes. Kidneys start to fail. Liver enzymes go wild. You might see jaundice, dehydration that doesn't respond well, and bloodwork that makes you wince. The gap between "mild" and "crashing" can be shockingly short.

 

So why do we stress early recognition so much?

Because I've seen dogs go from perky to critical in under 48 hours. You can't wait for all the classic signs to show up-they might not. And from a public health angle? If I'm even mildly suspicious, I'm already talking to the owner about urine gloves, disinfection, keeping the dog away from kids until we rule it out.

 

About that antibody test

The Canine Leptospira Antibody Test – basically, it's a blood test that checks for antibodies against Leptospira in dogs. It gives us a clue about whether a dog has been exposed to the bacteria or possibly infected.

In practice, vets use this test in a few different ways.

First, it's a handy screening tool. If a dog has been roaming around areas where leptospirosis is common, running this test can help flag potential exposure. Second, it comes in handy when we're dealing with sick dogs – you know, the ones showing fever, throwing up, acting lethargic, looking a bit yellow, or having kidney values that don't look right. The test results can give us one more piece of the puzzle. On a broader scale, it's also useful for keeping an eye on infection trends within a local dog population – sort of like a surveillance snapshot. And if you've got a dog that's always out hiking, swimming in ponds, or splashing through muddy puddles, this test can help assess its individual risk level.

Now, here's the catch – and it's an important one. A positive result doesn't automatically mean the dog has an active infection right now. Antibodies can stick around in the bloodstream for months after a dog has cleared an infection. Plus, if the dog has been vaccinated against leptospirosis, that'll trigger a positive result too. So, when we get a positive reading, we can't just stop there. We have to look at the whole picture – what's the dog actually showing clinically? What's its vaccine history? What do the other lab tests say? Only by putting all that together can we really make sense of what that positive result means.
